ArcGIS Experience Builder allows you to create web apps and pages using flexible layouts, content, and widgets that interact with 2D and 3D data. Experience Builder uses the Survey123 web app API so you can add a survey to your web app without writing any code. Use the Survey widget to add a survey. You can configure whether the user creates a survey record or selects an existing survey record to edit, the appearance of the survey, and where survey records are sent.
Prerequisites
The following steps require you to be working on an experience, though it does not need to be published yet. If you haven't started an experience, see How to use ArcGIS Experience Builder.
Steps
Use the Survey widget to add a new survey to your experience.

In ArcGIS Experience Builder click Insert tab.

Click and drag the Survey widget onto the layout.

Select Create new survey

Enter a name for your survey and click Create.

Drag and drop questions to your survey, configuring each question as required. For more information see Web designer essentials.

When your survey design is complete, click Publish.

The default survey mode is to submit new records. When a user opens the experience, they will see a blank survey that they can complete and submit as a new record. In Experience Builder click Publish.
What's next?
Optionally you can choose to show options, header, description, footer or theme of the survey. Enable or disable these features of the survey in the Survey widget content tab.
